Collaboration with our nonprofit implementing, government, and private sector partners helps to ensure care is coordinated across agencies to meet an individual’s needs.
Sandy was receiving hospice and being cared for at home by her son and daughter-in-law. APS (Adult Protective Services) contacted ISR to ask us to assist with locating respite and caregiver providers to help the family. We reached All Ways Caring (an in-home care agency), who went to work to find the needed assistance while ISR completed the necessary eligibility paperwork with APS. Because multiple coordinated agencies worked together behind the scenes, within a few days Sandy and her family had the help they needed.
